LIST OF REQUIREMENTS FOR ASSOCIATE MEMBERSHIP

• Application Form for Membership

– Filled up & signed by the Applicant
– Signed by the Proposer & Seconder (Both Proprietary Members)

• Assignment of Associate Member (Signed by the Assignor & Assignee)

• 1 x 1 size picture (white back ground) including dependents (2 copies each) for membership cards

• One Wallet Size Picture (of Applicant) for Posting.

• Birth Certificate of Dependents NSO Certified Copy

• No Cash requirements pay only monthly dues of P 4,000.00

Note :

Qualified in this category are dependents of Proprietary members who are 21 years old (on the birth date) up to 30 years old.
